Admission Process
Follow these simple steps for a smooth admission experience.
Doctor's Referral
Obtain a referral or admission order from your attending physician. This document should specify the reason for admission and recommended treatment plan.
Admissions Office
Proceed to the Admissions Office with the following documents:
- Doctor's referral or admission order
- Valid government-issued ID
- HMO card or insurance documents (if applicable)
- Letter of Authorization (LOA) from your HMO (if required)
Registration & Payment
Complete the admission forms and settle the required deposit. Our staff will explain the payment options and assist you with insurance verification if needed.
Room Assignment
You will be assigned a room based on availability and your preference. Our staff will escort you to your room and orient you to the facilities.
Important Guidelines
Visiting Hours
General Wards: 10:00 AM - 8:00 PM
Private Rooms: 8:00 AM - 9:00 PM
ICU/CCU: Limited visiting hours (please inquire)
Visitor Policy
Maximum of 2 visitors per patient at a time. Children under 12 years old are not allowed in patient rooms unless authorized by the attending physician.
What to Bring
- Personal toiletries and comfortable clothing
- Current medications (in original containers)
- Eyeglasses, hearing aids, or other assistive devices
Prohibited Items
- Smoking and alcoholic beverages
- Weapons or sharp objects
- Flammable materials
Emergency Admission
For emergency cases, proceed directly to our Emergency Department. Our emergency team will provide immediate care and handle the admission process. Required documents can be submitted later.
